Building Capacity for Successful Transition to Adult Life for Youth with Disabilities
This workshop is designed to enhance knowledge and skills in the development and implementation of person-centered planning systems for students with disabilities (emphasis on low-incidence). The term person-centered refers to an approach to assessment, goal setting and support that is designed from the student's point-of-view. A team of people who know and respect the student is gathered and engages in a facilitated group process that results in a plan for the future. The student and his team participate in a systematic problem-solving process that is documented. Regardless of the type of transition, person-centered planning can be utilized to assure a meaningful and successful future. Person-centered planning can be used to plan a variety of transitions including post-school life, entry to a new school or grade, or any other next step target.
